QVMC’s Outpatient Wound Care Program is the only outpatient wound care center located in Grant County. We specialize in chronic and hard-to-heal wounds. We treat diabetic foot ulcers, venous leg ulcers, and any other chronic wounds you or your patient may be suffering from. We will provide regular updates on your wound status to your referring provider and pride ourselves in working closely with our referring partners. Getting in to see us is easy – just have your provider fill out our referral form located on this page and fax it to us along with a face-sheet and recent chart notes relevant to your wound(s).
